This is, unfortunately, more common than you would think; it's certainly not the only post-mortem that I've done that had very similar findings. For example: "far too many of the deliverables, artifacts, and activities … are substantially below any acceptable professional standards and represent a profound threat to [the project] ever going into production."
- Integration migrates to the cloud | Software as Services | ZDNet.com – More ESB in the cloud, helping to pull together all those SaaS applications.
- Gartner Identifies the Top 10 Strategic Technologies for 2009 – BPM is off the list, but business intelligence and cloud computing are now on. Carried forward from last year: enterprise mashups and social software.
